Introduction: Digital health information systems are a key strategy for improving primary health care quality and efficiency. Objective: This study aimed to analyze the implementation of the e-Primary Health Center application in improving Primary Health Center performance in Merangin District. Methods: A qualitative case study was conducted through in-depth interviews, observations, and document reviews involving informants from the district health office and selected primary health centers. Data were analyzed through reduction, display, and conclusion drawing. Results and Discussion: Implementation was supported by adequate human resources, infrastructure, financial allocation, and district health office policy. The application improved data recording efficiency, reporting timeliness, and health service information management. However, challenges persisted, including unstable internet connectivity, variable digital literacy among health workers, and technical system performance issues. Conclusions: The e-Primary Health Center application has contributed to improvements in primary health center performance, yet sustained optimization requires strengthening infrastructure, enhancing user capacity, and ensuring reliable technical support across all implementing facilities
